We will send new patient intake forms to you electronically.
Arrive at least 10 minutes before your appointment time. Our clinic strives to run on time.
You may be rescheduled if you arrive late for your appointment.
Please dress comfortably. Sweat pants or leggings are ideal for individuals with low back or lower extremity complaints.
X-rays will be taken, if indicated. Sports bras are helpful to reduce prep time for radiographs.
The doctor will need time to review your exam information, x-ray listings and radiology report. Typically, a second visit will be required before treatment is given.
In most cases, multiple treatment encounters will be needed. A schedule of care recommendations will be provided by the doctor.
